At Canton Asthma & Allergy, we’re committed to making your first visit smooth, efficient, and stress-free. Below are important details about how to prepare for your visit. After you have scheduled your appointment, please take a few moments to review this information and help us deliver the quality care you deserve. To ensure a smooth and efficient visit, we provide all required patient forms online. For new patients, please download, print, and fill out the New Patient Packet below (includes the additional forms).
For existing patients, the individual forms are also provided below:
Insurance
Verify Coverage
Before your appointment, please check with your insurance provider:
- Is the office visit (CPT 99204 or 99205) covered?
- Is allergy skin testing (CPT 95004 or 95024) covered?
- Do you have a deductible that must be met?
- Is there a copay due at check-in?
- Is a referral from your primary care physician required?
- Is Dr. Steven Stryk in your network?
Important:
- If your plan requires a referral from your insurance, it must be received by our office before your appointment (BCN requires a global referral)
- If we do not receive your referral prior to your appointment, your visit will need to be rescheduled

We do not accept Medicaid.
Please verify your insurance coverage and referral requirements prior to your appointment.
Medications
To ensure accurate skin test results, please stop the following medications according to the following schedule before your appointment. You can call us at (734) 394-2661 if you’re unsure what medications to stop.
Visit Prep
What to Bring
Be sure to bring the following items with you to your appointment:
- Valid photo ID (Driver’s license or State of Michigan ID)
- Insurance card(s)
- Completed registration form
- List of medications
- Copay and any outstanding balances
Minors (under 18) must be accompanied by a parent or legal guardian with proof of guardianship.
Please do NOT wear any perfume or cologne to your appointment.
Cancellations & No-Shows
- Please cancel at least 24 hours in advance
- If you’re running late, call us at (734) 394-2661
- No-shows will be charged a fee and may lose future scheduling privileges
After-Hours & Emergencies
- For emergencies, call 911 or visit the nearest ER
- For non-urgent questions or refill requests after hours, call (734) 394-2661 and leave a voicemail
Note: Prescriptions are not filled after-hours, on weekends, or holidays.
Pay Your Bill
For your convenience, you can easily pay your bill online through our secure payment portal. Simply enter the required information to complete your payment quickly, safely, and efficiently.
You can also pay the bill by calling our office or mailing us a check.
