Questions

If you have HMO insurance, you will need a referral from your insurance. Blue Care Network (BCN) requires a global referral.

Please ensure our office receives it before your appointment or your visit will need to be rescheduled.

We do NOT accept Medicaid. Please check with your insurance provider for coverage options.

You must call your insurance company and find out your co-pay and your coverage.

Just bring a list of your current medications.

You can eat and drink before your appointment.

Patients under 18 must be accompanied by a parent or legal guardian for their appointment.

• Legal Guardians must bring official guardianship documents and a valid driver’s license for photocopying.
• Step-parents must provide legal adoption paperwork or ensure the biological parent has completed a proxy authorization form in the office in advance.

Other relatives must ensure the biological parent has completed a proxy authorization form in the office prior to the appointment.

Allergy shots need to be scheduled in advance. Allergy shots are given on Mondays and Wednesdays. The last allergy shot is given 30 minutes before closing time. Please call ahead before arriving at the office.

You MUST have your Epipen with you in order to receive your shot.

Prescription refill requests are processed only during regular office hours.

To request a refill, you must:

• Call our office and leave a message on the prescription refill line, or
• Contact your pharmacy, and they will send us an electronic fax request.

Please allow 48 hours for your request to be completed.

Important: If you have not had a follow-up visit within the past year, we cannot issue a prescription or refill. Your appointments must be up-to-date per Dr. Stryk’s instructions in order for prescriptions to be refilled.

Results from laboratory and imaging tests may come back at different times, sometimes taking up to two weeks.

We will notify you of your lab and imaging results by phone when they have all been completed.

Our office will complete and sign school forms provided by a parent or legal guardian during a scheduled visit.

• Bring your child’s school forms with you to the appointment.
• A fee may apply if forms are requested outside of an appointment.
• Please allow up to 5 business days for form completion.

Please ensure your child is up-to-date with their annual visits. Scheduling appointments early in the spring/summer is recommended.

Important: We cannot complete school forms if your child has not been seen within the past year.

Flu vaccinations are recommended annually, other vaccines vary and depend on age and health conditions. For more details, ask Dr. Stryk or a member of our expert team.

Asthma is diagnosed through a combination of history, physical exams, and pulmonary function tests.

Food allergy testing typically involves blood tests or skin testing to identify allergens. In some cases, we may recommend a food challenge under medical supervision to confirm an allergy has resolved.

We treat a variety of allergic conditions, including asthma, food allergies, pet allergies, drug allergies, skin reactions like hives, and environmental allergies like pollen or dust.
